To prepare you for your trip to Ecuador and the Galapagos we have created the Departure Center Confirmation Packet below. The documents and links presented will provide you with all the information you will need to prepare for your trip. By completing and then returning all pertinent forms to us in a timely fashion, you will guarantee that any special needs you may have are met to insure you enjoy a safe and well planned trip of a lifetime.
Important Note for Sending Forms:
We kindly request that you complete and submit your forms online rather than filling them out by hand. If you must fill out forms by hand, we prefer if forms are sent scanned or digitally photographed via email, but they can also be faxed to the following fax number in the United States: + 1 (925) 394-1138.

If you are unsure of which of the two itineraries you are booked on check your invoice, welcome email or ask your sales person. These documents are a day-by-day description of the activities and places that await you during your week in the Galapagos. If you are making arrangements with us in the mainland we will email an itinerary or link for that as well.
Completing, signing and returning your form is a requirement for participating in any of Quasar Expeditions' trips. Returning them in a timely fashion is also critical to insuring that you have a smooth and safe trip. We strongly recommend that you fill out and submit the forms online, but these can also be printed out, to be filled out by hand, or they can be filled out using a PDF editing program such as Adobe Acrobat.
Each traveler must fill out a separate form and the forms must be submitted online or returned to us either scanned or digitally photographed via email. They can also be faxed to the fax number provided in the first section of this page. We kindly request these forms are completed and returned at the earliest opportunity.
There are few places on earth as magical to a child as the Galapagos. This document helps insure the safety of children taking part on a Galapagos cruise by making sure parents and guardians understand and acknowledge the risks inherent in taking a minor on a trip of this nature. For those bringing minors (under 18) onboard, please read this online document carefully and then complete and submit it online. Alternatively, you can complete the PDF version, either by printing it out and filling it in by hand, or by using a PDF editing program such as Adobe, and then emailing or faxing us (to the number provided above) the completed and signed document.
We have long welcomed guests with disabilities on our cruises to the Galapagos Islands. The Galapagos, being a completely natural habitat, come with some challenges for those with disabilities. To insure a safe trip, this document helps to insure those parties so affected understand and acknowledge the challenges inherent in a Galapagos expedition.
Understanding Quasar Expeditions’ policies in the event of a cancellation or unforeseen occurrence is very important. Please make sure you read and understand all sections of this document. Important: By signing the Reservation Form, you are indicating that you have read and are in agreement with Quasar Expeditions' Booking and Cancellation Policies.
Many of the islands you will visit in Galapagos are remote, with limited or no access to immediate medical assistance. Therefore to insure against the high costs of medical evacuation we REQUIRE our passengers to purchase and show proof of medivac insurance. The minimum medical evacuation coverage requirement is US $50,000 per person. The cost of medivac insurance for this tripis relatively inexpensive, while the cost of medical evacuation can range well into the tens of thousands of dollars. This coverage protects your safety & finances.
A good Medivac policy will also provide medical coverage during your trip. The US State Department advises, "Before you go abroad, learn what medical services your health insurance will cover overseas… You are NOT covered by Medicare abroad." Medivac coverage can be useful in a number of ways.
This "travel" insurance covers all the non-refundable portions of your trip should you cancel or delay your trip due to illness, injury or other covered events. A good policy also includes medivac insurance and will also cover things like baggage loss, flight delays and more. This coverage is strongly recommended. Pre-existing conditions are typically covered if you purchase coverage within 24 hours of confirming your reservation.
